چکیده

What does this article tell us? Mark: This is a retrospective study of the safety of catheter-directed thrombolysis for proximal DVTs in 90,618 patients who were hospitalized between 2002 and 2010 (estimated to be about one-fourth of the cohort hospitalized during those years). Of these patients, 3,649 underwent catheter-directed thrombolysis. Those who received thrombolysis were compared with those who received standard anticoagulation after propensity matching. In terms of outcomes, all of the risk numbers favored not using catheter-directed thrombolysis. The rate of blood transfusion was 11.1% in the thrombolysis group vs. 6.5% in the anticoagulation group (odds ratio [OR] = 1.85; 95% confidence interval [CI], 1.57 to 2.20; P < .001), the rate of pulmonary embolism was 17.9% vs. 11.4% (OR = 1.69; 95% CI, 1.49 to 1.94; P < .001), and the rate of vena cava filter placement was 34.8% vs. 15.6% (OR = 2.89; 95% CI, 2.58 to 3.23; P < .001). Intracranial bleeding, length of hospital stay, and cost were also more favorable with standard treatment.

Thrombolysis in Stroke Patients; Problems and Limitations

Thrombolysis for stroke is being used in some developing countries. This study was designed to evaluate the problems of thrombolysis therapy in Iran. During January-July 2008, all patients with ischemic stroke admitted to Ghaem Hospital, Mashhad, northeast Iran, were enrolled in a prospective observational study. Ghaem Hospital is a tertiary care hospital that includes infrastructure for thromb...

Thrombolysis of deep vein thrombosis and inhibiting chemotaxis of macrophage by MCP-1 blockage.

OBJECTIVE Deep vein thrombosis (DVT) is one common vascular complication after trauma or surgery. Macrophage plays an important role in recanalization of thrombosis and monocyte chemotactic protein 1 (MCP-1) has a potent chemotactic role for macrophage. This study investigated the role of MCP-1 and macrophage in DVT thrombolysis. MATERIALS AND METHODS DVT mice model was established for evalua...

Prospective determination of candidates for thrombolysis in patients with acute proximal deep vein thrombosis.

PURPOSE To prospectively determine the distribution, extent, and age of venous thrombosis in patients presenting with acute signs and symptoms of venous thromboembolism and identify candidates for thrombolysis. MATERIALS AND METHODS Five hundred seventy-six consecutive patients (281 male, 295 female; mean age 58) referred for lower extremity deep vein thrombosis (DVT) assessment between Novem...

Clinical biomarkers and management of post thrombotic syndrome.

The post-thrombotic syndrome (PTS) is a long-term complication of deep venous thrombosis (DVT) that is characterized by chronic, persistent pain, swelling and other signs in the affected limb. PTS is common, burdensome and costly. It is likely to increase in prevalence, since despite widespread use of and improvements in the efficacy of thromboprophylaxis, the incidence of DVT has not decreased...

The role of thrombolysis in the clinical management of deep vein thrombosis.

The cornerstones of current management of deep vein thrombosis (DVT) are the routine use of anticoagulant therapy, graduated elastic compression stockings, and early ambulation. Thrombolytic therapy was previously reserved only for patients with life-, limb-, or organ-threatening complications. However, the postthrombotic syndrome has been increasingly recognized as a frequent and serious long-...
